Participation Waiver & Release of Liability Agreement

Please review this agreement carefully before continuing. By selecting "I AGREE," you confirm that you have read, understand, and voluntarily accept the terms of this agreement on behalf of yourself and/or the participating minor. I understand that participation in Plenty Progression and Plenty Shooters basketball training, skills sessions, practices, games, tournaments, camps, clinics, conditioning, and other related activities involves inherent risks. These risks may include falls, collisions, physical contact, strains, sprains, fractures, concussions, illness, serious injury, disability, and other risks associated with athletic participation. I knowingly and voluntarily accept and assume these risks on behalf of myself and/or the participating minor. TO THE FULLEST EXTENT PERMITTED BY LAW, I RELEASE AND WAIVE CLAIMS AGAINST PLENTY PROGRESSION, PLENTY SHOOTERS, THEIR OWNERS, COACHES, TRAINERS, STAFF, VOLUNTEERS, REPRESENTATIVES, AFFILIATES, EVENT ORGANIZERS, AND PARTICIPATING OR HOST FACILITIES FOR INJURY, ILLNESS, LOSS, PROPERTY DAMAGE, OR OTHER HARM ARISING FROM OR RELATED TO PARTICIPATION, INCLUDING CLAIMS CAUSED IN WHOLE OR IN PART BY THEIR OWN ORDINARY NEGLIGENCE, NEGLIGENT ACTS, OR NEGLIGENT OMISSIONS. I confirm that the participant is physically able to participate and agree to provide accurate information regarding relevant medical conditions, allergies, injuries, medications, or physical limitations. If an injury or medical emergency occurs and I cannot be immediately reached, I authorize reasonable emergency medical assistance and understand that I am responsible for resulting medical expenses. I agree that the participant and accompanying family members will follow all program rules, coaching instructions, safety procedures, facility policies, and standards of appropriate conduct. I understand that Plenty Progression and Plenty Shooters may restrict, suspend, or remove a participant from an activity when continued participation presents a safety concern or when serious or repeated violations of program rules occur. I understand that participation does not guarantee playing time, team placement, tournament participation, scholarships, recruiting opportunities, athletic improvement, or any specific result. I acknowledge applicable program policies regarding drop-off and pickup, supervision, transportation, personal property, memberships, payments, cancellations, credits, refunds, scheduling changes, and participation at third-party facilities and events. Photo & Video Release I authorize Plenty Progression and Plenty Shooters to photograph, film, and record the participant during training sessions, practices, games, tournaments, camps, clinics, and other program-related activities. I grant permission to use the participant's image, likeness, voice, and recorded content for lawful promotional, marketing, advertising, social media, website, educational, team, and program-related purposes without compensation. I understand that content published publicly or online may be shared or copied by third parties outside the organization's control. Electronic Agreement & Acknowledgment By selecting "I AGREE," I confirm that I have read, understand, and voluntarily accept this Participation Waiver & Release of Liability Agreement, including the assumption of risk, release of liability, medical authorization, program terms, and photo and video release stated above. If the participant is under 18 years of age, I certify that I am the participant's parent or legal guardian, or that I otherwise have legal authority to agree on the participant's behalf. I intend my selection of "I AGREE" to constitute my electronic acceptance of this agreement and to have the same legal effect as my signature to the fullest extent permitted by law.
